NOTES FOR GUIDANCE OF APPLICANT

1. An application under the Act must be in writing and it may be made either direct to the former employer or through a local office of the Ministry of Labour. If made through a locar office, it must be made in this form. The application must be made not later than the fifth Monday after the end of war service, unless the applicant was prevented from making it within that period by sickness or other reasonable cause, and the application was made as soon as reasonably may be after the expiration of that period.

If the undertaking in which the applicant was last employed (see Note 3) has been taken over by another employer, the application should be addressed to that employer. Applicants who seek reinstatement as merchant seamen should apply to the Shipping Federation Limited (Merchant Navy Reserve Pool) and not to a particular employer. Such applications should be sent to the Superintendent of the appropriate Mercantile Marine Office, or lodged at a local office of the Ministry. Applicants who seek employment under dock labour schemes and who are in doubt as to whom they should apply are advised to lodge their applications at a local office of the Ministry.

2. Valid applications can be made only by the following persons :-

(a) Male persons who, after 25th May, 1939, enter upon a period of whole-time service in the armed forces of the Crown.

(b) Female persons who, after 25th May, 1939, enter upon a period of whole-time service in any of the following capacities :—

Member of Queen Alexandra's Royal Naval Nursing Service or any reserve thereof.
Member of the Women's Royal Naval Service.
Woman medical practitioner serving in the Royal Navy or any naval reserve.
Member of Queen Alexandra's Imperial Military Nursing Service or any reserve thereof. rank as an officer.
Member of Princess Mary's Royal Air Force Nursing Service or any reserve thereof.
Member of the Women's Auxiliary Air Force.
Woman employed with the Medical Branch or the Dental Branch of the Royal Air Force with relative rank as an officer. Member of the Voluntary Aid Detachments employed under the Admiralty, Army Council or Air Council.

(c) Persons (whether male or female) who, after 10th April, 1941, enter upon a period of whole-time service in consequence of an enrolment notice under the National Service Acts, 1939 to 1942, in the National Fire Service, Police War Reserve or Civil Defence Reserve.

3. Last employment before beginning war service.

Give information about the last employment within the period of four weeks immediately preceding the beginning of war service.

4. Particulars of whole-time service.

Under item 4 (a) state briefly to which of the whole-time services mentioned in Note 2 above the applicant belonged, e.g., R.N., Army, A.T.S., N.F.S., etc. Under item 4 (b) state the date that whole-time service began. if the service was in a civil defence force, the date must be the date of enrolment under the National Service Acts. Under item 4 (c) state the date the applicant went on final leave pending release or discharge. If no such leave was granted, state the date when applicant's whole-time service ended.

5. Particulars of certain further whole-time service.

This section of the application form should be completed only if the period of whole-time war service shown in section 4 was followed by a further period of whole-time service of the kind mentioned in Note 2 above, or by whole-time civilian service which the applicant was released or discharged from the forces or women's services to perform, or which was performed by direction or written request of the Ministry of Labour.

6. Notification of availability for employment.

If possible, state when available for work. The date must be not later than the ninth Monday after the end of war service, unless delay arises owing to sickness cr other reasonable cause. If there should be such delay, notification of availability should be made as soon a possible. If no date is stated on this application form, the employer should be notified later and within the period allowed.

7. Other remarks.

If the application or the notification of availability for work is delayed owing to sickness or other reasonable cause, the reason should be stated in section 7 overleaf. Any other matters to which the applicant wishes to draw attention can also be stated in section 7 (e.g., special skill, disability, etc.).
